Midtier Sign Up Procedures

Please read all information before completing the required documents.

Please note Midtier forms are due no later than April 17, 2009 to allow ample time to complete registration for any additional classes for the Fall 2009 semester. You may turn in your forms any time prior to that date once your fall schedule is complete. All forms must be turned in together to Ronan 204 and must be in no later than April 17, 2009. (Note: The due date for Summer 09 is March 6, 2009)

  • As a Midtier student you will be filling out four important forms for field placement for the Fall 2009 semester, the Midtier Student Information Placement Sheet, Student Autobiography, the Public Relations Promotional Release form, and a copy of your ICHAT (Internet Criminal History Access Tool) through the Michigan State Police. The forms must be typed. The Student Autobiography form must be written in complete sentences. Print 2 copies of the Student Information Placement Sheet and the Student Autobiography, and one copy of the Public Relations Promotional Release form, and include a copy of your ICHAT through the Michigan State Police. Bring all forms to Ronan 204 by 5:00 p.m., April 17, 2009.
  • Please note the due date for the required Midtier forms is April 17, 2009. This will allow time for additional classes to be added to your schedule through SAP. Also, this gives ample time to be sure your schedule is accurate and inclusive of all classes for the fall, 2009 semester. You may turn your forms in anytime prior to the due date.
  • On the designated area on the form, please indicate the day and 4-hour time block either in the morning or afternoon (Note: you must fill out the semester course schedule, printed class schedules from SAP will not be accepted). Four hours would include either 8:00 AM to 12 noon or 12 noon to 4:00 PM. This block MAY NOT INCLUDE time over the lunch hour. Every effort will be made to secure your first choice of day and time, but please note there is NO GUARANTEE that you will be assigned this time and day. You may not do your Midtier experience in a district you have attended and all placements are made by the Midtier coordinator.
  • Please include any work hours that would impede your ability to accommodate certain blocks of time.
  • If you are commuting from an area outside of Mt. Pleasant and would prefer to obtain your Midtier hours closer to your home, please list two school choices in the additional information section of your placement form. Please remember you may not obtain your Midtier hours in a district you have attended. You also may not obtain your hours in a school building where you have any relatives working or attending the school.
