Each visit, the host teacher will initial and verify the Midtier student attendance form.

Secondary Education Field Requirements
Due Dates Fall 2009:

Week of August 24, 2009

  • Midtier coordinator in attendance at university class
  • Distribute student Midtier packets
  • Discuss school placement information

 Week of September 21, 2009

  • Begin Midtier field experience in PK-12 classroom
  • Begin keeping record of attendance
  • Begin journal entries on Blackboard

Week of October 5, 2009

  • Midtier coordinator in attendance at university class
  • Complete Placement Verification assignment on Blackboard
  • Questions/concerns on field placements

Week of October 26, 2009

  • Midtier coordinator in attendance at university class
  • Mid-Term evaluation form due
  • First lesson evaluation form due with typed lesson plan
  • Discussion of questions or concerns
  • Submit first set of journal entries by October 30, 2009

Week of November 30, 2009

  • Midtier coordinator in attendance at university class
  • Final evaluation form due
  • Second Lesson evaluation form due with typed lesson plan
  • Initialed, totaled and signed attendance record due
  • Submit second set of journal entries by December 2, 2009
  • Diversity Check List assignment to be completed on Blackboard
  • Midtier experience survey form to be completed online. The link will be provided in an email through global ID.
