Dear Parents/Guardians,

Congratulations on choosing the Human Growth and Development Laboratory as your child's preschool.  To begin enrollment the following information is required for your child to attend our preschool.

A copy of your child's birth certificate A copy of your child's immunization record
A current physical form completed by your child's physician A completed Application
Your child's physician's phone number and address HGDL Parking Permit Request
Health Insurance information Family Involvement Information
Child Placement Contract ICTC - if you wish to have your child transported
C.A.C.F.P. Follow-Up Permission Form
Proof of Income may be required
